Due to complications by the company that does our scheduling, all mediation forms have changed to a default language of "First Name" and “Last Name" instead of Petitioner/Plaintiff and Respondent/Defendant full names. Also, it should say Case Number not "Phone". Below is an example of what the change looks like. In Black is the information needed in each box.
